Medical Records Clinical/ Formulary Management Medicare Part D Our Pledge

All Medicare beneficiaries are eligible for a Medicare prescription benefit, referred to as Medicare Part D.  Beneficiaries are required to enroll in a Prescription Drug Plan (PDP) or Medicare Advantage (PPO) plan. Everyone with Medicare, regardless of income, health status, or prescription drugs used, can get prescription drug coverage.

Long Term Care and Assisted Living residents who are dually eligible for both Medicare and Medicaid benefits, are facing new challenges such as formulary management. Partners is a leader in assisting the facilities we serve with streamlined processes for Med D utilization.
